Shinpachi might hate the way people underestimate him because of his height, but he knows full well that he can use it to his advantage. Watching his attacker go from cocky to fearful (and occasionally to dead) gives Shinpachi a deep sense of fulfillment.
He knows Okita feels the same about kicking the ass of someone who thought he was too feminine to be a competent swordsman. Okita is scary in a way Shinpachi isn't though, Okita lives and breaths for the sword, and some day he will die on the sword. Shinpachi is perfectly fine living and breathing for his friends, for good sake and festivals, for good cooking, and for a pair of strong arms to welcome him into his futon after a long day.
And Shinpachi knows that one day he will die for those things too.
